Chronic disease-specific education is for people living with the following chronic conditions:
- Diabetes
- Stroke
- Heart Disease
- COPD
- Asthma
Education:
- Education is specific to the individual and his/her family.
- Educators can refer to many other community programs and professionals.
Do You Need a Referral?
No. You can refer yourself to the program or you can be referred by your physician or other health care professional.
