
Who Is it For?
Heart to Heart is an education and support program for cardiac patients and their families. Heart to Heart is helpful for any person who has had a heart attack, heart surgery, or angina, or who is at risk for heart disease.
Please feel free to bring a partner or other support person. Family members have questions too!
Heart to Heart is offered in a series. Sessions feature a video, informal group discussions, and activities. All sessions are designed to help make living with heart disease easier for you and your caregiver.

Do You Need a Referral?
No. You can refer yourself to the program or you can be referred by your physician or other health care professional.
