Who Is it For?
The program is for caregivers and/or individuals who have one or more chronic health conditions such as (but not limited to):
- Cancer
- Chronic Pain Arthritis
- Diabetes
- Heart Disease
- Chronic Lung Disease
- Parkinson's Disease
- Stroke/Spinal Cord Injury
- Osteoporosis
- Multiple Sclerosis
- Kidney Disease
The program began in 2002 and is delivered under license from Stanford University by volunteer lay leaders. Pairs of trained volunteers work with groups of up to 12 people, once a week for 2.5 hours for six consecutive weeks. Results suggest that the program impacts people's sense of well-being and reduces healthcare utilization. The LiveWell With Chronic conditions program is available throughout the province.
Goals of the Program:
To help participants obtain information, learn new skills and abilities and develop higher levels of confidence to manage and cope with chronic health conditions.
To give people increased confidence in handling their own health problems.
To give and receive support from others who are experiencing similar health problems.
Service:
Included in this six-week program are:
- Problem solving and action plan
- Exercise
- Cognitive symptom management
- Nutrition
- Fatigue and symptom management
- Advanced care directives
- Use of medications
- Dealing with fear, anger and depression
- Communication with others
Cost:
This program is offered Free of Charge.
Schedule:
2.5 hours weekly for six consecutive weeks.Class dates change with each session. Please call to inquire about specific class start dates.
Location:
The program is held in locations throughout Prairie North Health Region dependent on interest.
