
Who Is it For?
Stroke Rehabilitation Clinic is for people who have suffered a stroke and need continued therapy. The sooner you begin rehabilitation following a stroke, the more likely you are to regain lost abilities and skills.
Rehabilitation Team:
- Physiotherapist - to assist with strength, function, movement, walking, and pain management
- Occupational Therapist - to assist with maximum independence in activities of daily living and self care including productivity and leisure
- Speech and Language Therapist - to assist with speech, understanding, or difficulty swallowing
- Acquired Brain Injury Coordinator - To meet with survivors of brain injury and their families to assist in linking with services needed to continue to live successfully in their communities.
- Chronic Disease Nurse - to assist with education about stroke and help navigate the healthcare system
- Other Team members - on an as-needed basis, such as dietitians, social workers, etc.
Do You Need a Referral?
No. You can refer yourself to the program or you can be referred by your physician or other health care professional.
